Client Forms: Why do I need to answer all those questions?

Filling out forms isn’t the most fun thing to do but every question I ask on a consultation form gives me a clue to how to best treat your skin.

Your skin is affected by your genetics, your occupation, your lifestyle, your geography, your food choices, and also the products you use and how you use them. Having some insight into these topics helps me ensure that your treatments and products are safe and effective.

When treating skin conditions, I have to be a little bit of a detective to try to find what is triggering a reaction in the skin. It can be toothpaste, sun exposure, a food item or how you are eating a food item, shampoo and other hair care products, medications, the list goes on and on. Sometimes I have to ask someone to walk me through their day, or their skincare routine, in order to find the spot where we can make an improvement.

When you fill out the consultation form it’s also important that you’re honest. I know you probably have sunscreen but are you actually putting it on your face every day? The one that works the best is the one that you’ll use. No judgments about an incomplete routine or not having the best habits (we’ve all been there) but I need to know where you’re at and what you’re willing to change to actually create improvements in your skin.

A key question on my forms is about what changes you would like to see in your skin. Lots of people have multiple concerns some might really bother them and others may not at all. What I would try to improve might be the one that you don’t care about so knowing what will make you feel more confident and beautiful is very important to me.

I also ask about medical history and medications as they can have a direct impact on the skin or how sensitive or reactive your skin may be. I believe that gentle products can also be the most effective products but even water can sting skin that is barrier impaired and sensitized. There are medications that can make your skin extremely dry and/or thin and that would definitely change the way I think about how to treat you.
